Seeing it happen often when there's no logical reason why.
Seems to be only on Offense but the game situation doesn't dictate a valid reason.

Anyone else noticed this ?

When it happens to one of my teams I figure it's just a miscommunication or something from the sidelines that takes too long to figure out before the play clock expires. So a timeout is called. It happens once in a while even in real life. I haven't seen it a lot so I haven't been too alarmed.

If you were a Packers fan you would see it on a weekly basis...

I guess my teams' headsets work okay and haven't been messed with. :)
